Beginning during the Third Doctor's era references are made fairly commonly to the fact that which ever number the actor is, that is the incarnation the Doctor is (ie. the third actor to play the Doctor is the third incarnation of the Doctor, etc.) Also, just to clarify things a little... The Doctor has 12 regenerations. That means he has 13 incarnations. One original life, and 12 renewals (to use the original term the Second Doctor used to describe what happened to him). So no, he hasn't used up all of his incarnations. He still has three (soon to be two) left. However, the Master has been offered a new cycle of regenerations twice, and actually been given them once by the Time Lords, so who knows what could happen if the show lasts long enough for the Doc to use up his last few regenerations in his current cycle. Your husband is off a bit, but I think what he's referring to is the Original Series story "Trial of a Time Lord", which was a Colin Baker story, not a Tom Baker story. There's a character in the story called "The Valeyard" who the Master describes as being an entity created from the Doctor's darker impulses "between his 12th and final incarnations". His secret purpose in "TOATL" is to destroy the Doctor and inherit his remaining regenerations with the help of corrupt members of the Time Lord High Council.
