I think you might try a combination of the techniques here, along with good old-fashioned self-responsibility.

Let him know that it is his body, and his body has a problem (see Jenny's post) and that the Bible says that we are to love our bodies and take care of their needs. You and him are on the same team, to try and find a way to get him healthy again. It's not fair that his tongue and his stomach are working against the health of the rest of the body. It's like a bully standing at the classroom door and not letting a doctor come in with medicine for everybody. You & him need to find a way to get the medicine down no matter what his tongue thinks, then try to stay calm and think of other things so his stomach doesn't react. You have some ideas... does he have any?

Be highly supportive and welcome creativity. If he thinks he'll feel better taking it in the fresh air (to help with the nausea) at the airport near the end of the runway so he can distract himself watching a plane take off... that's a good idea! Off you go after supper to the airport. If he thinks a reward would help his will power, think outside the box... firecrackers? Sure! (Presuming they are legal)

Have him choose a plan (or plans) and commit to it, to be the winner against his tongue and stomach. Do whatever he wants (within reason) to support him. If he refuses to, continue to make it his problem by applying discipline. God trusted you to parent him and keep him healthy. You are not allowed to just let him not take his medicine. If he won't do it willingly, he's going to have to... be in time out, loose privileges, do extra chores... whatever's normal in your house... until he becomes a willing participant in keeping his body healthy.

My 3 year old has been on a lot of medications in the last 6 months and what seems to work best for him is to use the plastic syringe for liquid medications. It goes down much faster then a spoon and you can even squirt it over his tongue. Chase with his favorite drink. Acidic drinks (like the grapefruit one mentioned or Coke) help cleanse the tongue. Check your pharmacy for the syringes. I had to ask.

Some medications also work better with thick mediums (like mentioned before). Pudding, applesauce, and yogurt work well. Take a spoon full of the medium then mix in the medicine well. Do not mix it throughout a bowl. There's nothing worse then making them eat every bite if they catch a taste. This process works well for meds with odd textures that don't go down well.

Make it part of a special treat. If he has a special drink or ice cream to follow (only his) it helps him have something to look forward to.

Praise him a lot, even just for trying.

Since you are dealing with a boy, you might want to alter the speech I posted earlier to bring it along the military vein. In other words, he has an army of bacteria trying to take over his body and he has to be a good soldier and fight back. Make him a pin to wear for each successful day, let him sleep in a tent, paint his face, whatever appeals to him!

I've used that type of reasoning. I guess you missed this line from the first post.

He knows he needs the antibiotic, he is not opposed to antibiotics, per se, it is simply the experience with getting continually sick due to the bad taste that he doesn't want liquid antibiotics.

UPDATE:
I spoke with my son's father today. He said that he pretty much force-fed the first dose to ds, after that, ds was willing to take the med with only a little protest because he realized that this one really WASN'T as bad as the first.

I would presume so, but the pediatrician won't prescribe pills to him because he is too light.

My daughter gets that a lot during cold and flu season and you are right it is NASTY I've tried a taste and almost threw up. I really don't understand how they think kids will willingly take it. We are lucky(kinda) she has a feeding tube started out to get some lbs on her now its just used for meds so we can bypass the taste stuff.
Sorry I don't have any suggestions for the OP.
